Job summary

Fundamental is seeking a Licensed Social Worker (LSW) to oversee social services within a facility, coordinating with discharge planners and families to schedule facility tours and follow-up procedures. The role requires a current Social Work license, a Bachelor's in Social Work, and at least 3 years of experience in a long-term care setting.

Qualifications

• Must have current Social Work license in good standing with the state
• Bachelor’s Degree in Social Work strongly preferred
• Must have at least 3+ years of experience in a long term care facility
• Must be able to organize and prioritize many tasks effectively
• Ideal candidate must be a skilled communicator, director and motivator
• Reliable transportation required

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing of the Social Services Department within the facility in accordance with current existing Federal, State and local standards
• Develop and manage the initial contact with the discharge planner, family or responsible party to initiate tour of facility and follow up procedures
• Assist in developing and implementing policies and procedures for identifying the medically-related social and emotional needs of the patient/resident
• Develop and maintain a good working rapport with intra-department personnel, other departments within the facility and outside community health, welfare and social agencies to ensure social service programs meet patient needs
• Keep abreast of current federal and state regulations as well as professional standards
