Job summary

Cross Healthcare Services is seeking a Social Worker to deliver comprehensive social work and discharge planning for a targeted patient population. You will collaborate with physicians and the interprofessional team to address psychosocial needs and ensure smooth transitions of care.

The role requires LICENSURE as an LMSW in Texas and experience with discharge planning, patient assessments, and care coordination to improve outcomes and length of stay.

Qualifications

• 1+ year of social work in a health care setting strongly preferred.
• Licensed Master Social Worker (LMSW) or higher license in the State of Texas.
• At least three (3) years of accounts receivable management experience in health care.

Responsibilities

• Communicates effectively with health care team and reports patient data in a comprehensive manner.
• Facilitates discharge planning activities and coordinates with case managers and interprofessional team.
• Owns psychosocial assessments, diagnoses and treatment within discharge planning.
• Assesses patients for care transitions, identifies barriers to throughput, and supports family coping.
• Completes timely assessments for unfunded patients to identify community resources.
