Responsibilities

• Plan, organize, develop, and direct the Social Services Department to meet residents' emotional and social needs.
• Plan, develop, organize, implement, evaluate and direct social service programs.
• Participate in discharge planning and resident assessments.
• Interview residents/families in private settings.
• Refer residents to appropriate social service agencies as needed.
• Inform residents/families about Medicare/Medicaid and other financial assistance programs.
• Perform additional responsibilities as needed.
