Qualifications

• MSW degree and Texas licensure as a Licensed Social Worker.
• 2+ years in community health, case management, or clinical care.
• Bilingual in Spanish with ability to travel within region.

Responsibilities

• Conduct comprehensive assessments (telephonic or in-person) to identify physical, behavioral, and social needs.
• Develop, implement, and monitor individualized care plans with members and families.
• Coordinate care for members with chronic conditions or high-risk needs.
• Facilitate referrals to social services and community resources.
• Educate members on care options and available services.
• Serve as primary contact for care coordination and advocacy.
• Ensure timely documentation of assessments and services.
• Participate in interdisciplinary team meetings and transitions of care.
