COPE Health Solutions seeks a Licensed Social Worker to join the Care at Home Solutions program in a hybrid LA office. You will conduct comprehensive behavioral health assessments, provide counseling, develop care plans, and coordinate with a multidisciplinary team to improve patient outcomes.

The role requires active licensure in CA (LCSW or equivalent) and 3–5 years of care management experience, with strong collaboration skills and knowledge of community resources.

Qualifications

• Valid and current LCSW, LMHT, LPCC, LMFT, LCADAC, LICSW, or LMSW CA licensure
• 3-5 years’ care management and/or managed care experience in one of the listed settings
• 2-5 years of leadership experience in care management/managed care (preferred)
• Bilingual Spanish/English strongly preferred.

Responsabilités

• Assess patients to determine appropriateness for management early in their disease process.
• Develop a care plan and utilize judgement to evaluate needs for services.
• Assess Social Determinants of Health (housing, food, transportation, safety).
• Collaborate with APPs, physicians, pharmacists, nurses, and CHWs to address needs.
