The role requires 2+ years in health care, LTC/ LTSS experience, and travel up to 40% to member homes and provider locations.

Qualifications

• 2+ years of health care experience, including LTSS or care management.
• Licensed/certified only where required by state contracts or mandates.
• Valid driver's license and reliable transportation for travel.
• In some states, a bachelor's degree in a health care related field may be required.
• Strong communication and interpersonal skills.

Responsabilités

• Complete member assessments within regulated timelines, including home visits.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members and care teams.
• Monitor and adjust care plans to meet goals.
• Coordinate LTSS and behavioral health services for seamless care.
• Travel up to 25-40% for local/state requirements.
