UT Southwestern Medical Center in Dallas is seeking a licensed master’s level social worker to provide coordinated home-based care, develop patient-centered action plans, and promote education, self-management, and adherence to the PCP plan.

The role emphasizes assessing home environments, linking patients to community resources, coordinating discharge planning, and contributing to quality improvement initiatives. A Texas social work license and a master's in social work are required.

Competenze

• Master's Degree in social work.
• 1 year of experience in a health care setting preferable with homebound patients.
• Current license is required by the Texas State Board of Social Work Examiners. (DRIVERS) TX Driver's License and proof of driver's insurance

Mansioni

• Ensures the delivery of quality social service for patients by identifying patient needs and developing social service programs to meet those needs.
• Participates in development and update of Plan of Treatment.
• Evaluates patient and home environment to determine social and psychological factors related to patient's health problems.
• Prepares and submits to agency psychosocial histories, clinical and progress notes.
• Counsels' patient and/or family in understanding, accepting, and adjusting to patient's health problems and emotional care.
• Identifies, informs, and refers patient/family to available community resources.
• Coordinates discharge planning.
• Interprets to the community the role of social work in the Agency's programs.
• Participates in planning, development, and delivery of staff in-service education programs.
• Participates in Continuing Quality Improvement Program. Monitors adherence to policies, procedures, and protocols.
• Performs other related duties as assigned or requested.
