Job summary

UnitedHealth Group in El Paso, TX is seeking a licensed social worker to join our Optum Health Enterprise Clinical Services team. The role focuses on managing non-HCBS waiver member care to improve outcomes and satisfaction, within a hybrid work model with field visits.

You'll develop and monitor individualized care plans, coordinate care for high‑risk patients, and partner with families and providers to connect members to community resources. Texas licensure is required.

Qualifications

• Master's degree in social work (MSW) and Texas licensure as a Licensed Social Worker.
• 2+ years of experience in community health, case management, or clinical care settings.
• Experience with STAR+PLUS Texas Medicaid managed care program.
• Proficiency with MS Word, Excel, and Outlook.
• Bilingual in Spanish.

Responsibilities

• Conduct comprehensive assessments to identify physical, behavioral, and social needs.
• Develop, implement, and monitor individualized care plans with members, families, and providers.
• Coordinate care for members with chronic conditions, disabilities, or high-risk complications.
• Facilitate referrals to social services, community resources, and support programs.
• Educate members and families on care options and available services.
• Serve as the primary point of contact for care coordination and advocate for member needs.
• Ensure timely documentation of assessments, care plans, and service delivery.
• Participate in interdisciplinary team meetings and support safe transitions of care.
