Job summary

TEEMA is seeking a compassionate Social Worker to provide comprehensive case management and psychosocial support to patients and their families in San Francisco. Under supervision, you will deliver assessments, care planning, counseling, and referrals to community resources to enhance independence and quality of life.

Responsibilities include independent clinical practice, collaboration with caregivers and multidisciplinary teams, documentation, and advocacy to access services.

Qualifications

• Master’s degree in Social Work (MSW) from an accredited institution required.
• Bachelor’s degree in Social Work may be considered with relevant experience and language proficiency.
• California Social Worker license or eligible required for practice in CA.

Responsibilities

• Exercise independent judgment to provide case management services for a panel of patients.
• Conduct psychosocial assessments to develop, implement, and update individualized care plans.
• Collaborate with caregivers and support systems to deliver coordinated care plans.
• Identify psychosocial risks, advocate, refer, coordinate, and follow up on patient support.
• Provide individual counseling to patients.
• Facilitate family conferences and patient/caregiver support groups.
• Engage with the community to identify and access services for patient wellbeing.
• Discharge planning and coordination when patients transition out of the program.
• Participate in caregiver workshops and educational sessions.
• Serve as liaison between patients, caregivers, the team, and community partners.
• Adhere to ethical and legal standards; comply with organizational policies.
• Complete documentation promptly and accurately.
• Deliver compassionate, respectful, professional service.
• Perform other duties as assigned.
