Qualifications

• MSW from an accredited institution required.

Responsibilities

• Provide psychosocial assessments and care coordination for a caseload.
• Deliver counseling and referrals to community resources as needed.
• Collaborate with caregivers and inter-disciplinary teams for coordinated care.
• Identify psychosocial risks and advocate for patient support.
• Conduct discharge planning and coordinate transitions to other services.
• Maintain timely documentation and uphold confidentiality at all times.
