Job summary

TEEMA seeks a compassionate Social Worker in San Francisco, CA to provide comprehensive case management and psychosocial support to patients and families, under supervision. You will conduct psychosocial assessments, care coordination, counseling, and referrals to community resources to promote independence and quality of life.

This role requires collaboration with caregivers and healthcare partners, documentation, and active participation in team meetings to improve service quality and outcomes.

Qualifications

• MSW from an accredited institution required.
• Bachelor’s degree in Social Work may be considered with relevant experience.
• Minimum of one year of experience working with frail or elderly populations.
• Valid Social Worker license in California (MSW – Medical Social Worker) required.

Responsibilities

• Provide case management for a panel of patients.
• Conduct comprehensive psychosocial assessments and develop individualized care plans.
• Collaborate with caregivers and support systems to deliver coordinated care plans.
• Identify psychosocial risks and service needs; advocate, refer, coordinate, and follow up on patient support.
• Provide individual counseling to patients as needed.
• Engage with external agencies and community resources to support patient well‑being.
• Complete documentation promptly and accurately.
• Actively participate in team meetings and contribute to collaborative care planning.
