Job summary

Regal Care at Wakefield is seeking a Licensed Social Worker to support residents’ emotional and psychosocial well-being in our premier skilled nursing and rehabilitation center.

The role involves collaborating with residents, families, and the interdisciplinary team to assess needs, develop care plans, provide counseling, and assist with discharge planning, all while upholding quality, dignity, and regulatory compliance.

Qualifications

• Bachelor’s Degree in Social Work (BSW) required; Master’s Degree (MSW) preferred.
• Current NJ Social Work license (LSW or LCSW) required.
• Experience In Long-term Care Or Sub-acute Rehabilitation Setting Preferred.
• Strong interpersonal, communication, and organizational skills.
• Compassionate and resident-centered approach to care.

Responsibilities

• Conduct psychosocial assessments for new admissions and ongoing residents.
• Develop and implement individualized care plans in coordination with the interdisciplinary team.
• Provide counseling and emotional support to residents and families.
• Assist with discharge planning and coordination of community resources and services.
• Serve as a liaison between residents, families, and facility staff.
• Document all assessments, interventions, and care plans accurately and timely.
• Advocate for residents’ rights and ensure compliance with state and federal regulations.
