Job summary

Forest Hill Center for Rehabilitation and Healing in Newark, NJ is seeking a Speech Language Pathologist (SLP) to provide diagnostic and therapeutic services for residents with swallowing or communication disorders. You will evaluate residents, document care, and participate in interdisciplinary team meetings.

The ideal candidate holds a current SLP license in the state, ASHA CCC-SLP or equivalent, with EMR experience.

Qualifications

• Current license as a Speech Language Pathologist in the state of practice.
• CCC-SLP from ASHA or equivalent certification path.
• Experience working with residents in healthcare setting and documentation.
• Proficiency with EMR software.

Responsibilities

• Evaluate residents’ functional communication or swallowing needs.
• Follow physician orders and facility policies to evaluate and treat residents.
• Assist DON in policies for communication/swallowing decline prevention.
• Participate in discharge planning when appropriate.
• Maintain resident reports for regulatory agencies and payers.
• Provide education to residents and caregivers.
