Job summary

Eagle Rock Post Acute Care at Cedar Grove is seeking a Speech Therapist (SLP) to join our rehabilitation team. We offer full-time, part-time or PRN shifts in a Skilled Nursing Facility setting, focusing on communication and swallowing therapies.

The role requires New Jersey SLP licensure, experience in long-term care, and strong organizational and communication skills to coordinate with therapy teams and documentation in electronic records.

Qualifications

• New Jersey SLP License is required.
• Experience in long-term care, specifically SNF setting.
• Strong organizational, interpersonal and communication skills.
• Ability to manage patients with varying personalities and ability levels.
• Creative problem solving skills.

Responsibilities

• Collaborate with therapy teams and Rehab Department.
• Complete evaluations and all necessary documentation.
• Provide recommendations supporting communications and assessments.
• Deliver speech therapy per treatment plans and prescriptions.
• Help improve patients' speech, dysphagia and overall communication.
• Independently manage workflow and treatment documentation in EHR.
