Job summary

Forest Hill Center for Rehabilitation and Healing in Newark, NJ, seeks a Speech Language Pathologist (SLP) to provide diagnostic and therapeutic services for residents with swallowing or communication impairments. You will work with an interdisciplinary team and contribute to policy development and discharge planning.

Candidates must hold a current SLP license in the state, ASHA CCC or equivalent, and have experience with EMR systems.

Qualifications

• Current license as Speech Language Pathologist in the state of practice.
• ASHA CCC or equivalent certification, or equivalent path to certification.
• Experience with documentation and care planning in SNF settings.

Responsibilities

• Evaluate residents’ functional needs and refer as required.
• Follow physician orders and document care provided.
• Assist in policy development for communication and swallowing decline prevention.
• Participate in discharge planning when appropriate.
• Perform diagnostic tests and document evaluations per policy.
• Provide treatment to maximize communication and swallowing function.
• Educate residents and caregivers to maintain or improve function.
• Maintain timely resident reports for regulatory and payers.
