Job summary

PACS is seeking a Licensed Physical Therapist Assistant to provide therapy services in the facility, following the physician treatment plan and PT guidance. You will maintain treatment notes, support staff training, and collaborate with the health team to optimize patient recovery.

Proficiency with Casamba and Point Click Care is required, along with strong reading and policy comprehension skills. The role emphasizes interdisciplinary communication and adherence to state licensure requirements.

Qualifications

• Licensed as a Physical Therapist Assistant in the state.
• Proficient in Casamba and Point Click Care.
• Ability to read and comprehend procedures and policies.

Responsibilities

• Record daily treatment notes and weekly progress notes per PT Board.
• Assist in maintaining department.
• Participate in Patient Care and Rehabilitation Conferences, as needed.
• Assist with cleaning and maintenance of treatment area.
• Treat patients per the physician treatment plan.
• Assist nursing department with training of Restorative Aides.
• Communicate with supervisor and other health team members regarding patient progress, problem and plans.
• Instruct patient’s families or nursing staff in maintenance program and caregiver training in preparation for discharge from therapy services.
