Job summary

Bloomingdale Nursing & Rehabilitation Center is seeking a licensed Physical Therapist Assistant (PTA) to assist in implementing the plan of care for residents. You will work as part of an interdisciplinary team to deliver rehabilitation services in a supportive, patient-centered environment.

The role requires an associate degree in PT/OT and current state licensure, with preferred experience in older adult rehabilitation.

Qualifications

• Licensed/certified Physical Therapy Assistant in the state of employment.
• Graduate with an associate’s degree in physical therapy or occupational therapy.
• Experience with older adult rehabilitation preferred.

Responsibilities

• Provide resident rehabilitation care per plan of care and standards.
• Communicate resident condition and responses with supervising therapist.
• Perform required documentation and charting.
• Support therapists and rehabilitation leadership in care delivery.
• Assist with center processes and training as requested.
• May travel to other facilities to provide treatments.
