Personal Healthcare Management LLC is seeking a licensed Physical Therapy Assistant in New York to implement PT plans under supervision, deliver skilled treatments, and support residents in a care setting.

The role requires an Associate’s degree in Physical Therapy, a NYS PTA license, and the ability to work independently or with a team; geriatrics experience is preferred and transportation is needed.

Formación

• Must possess a current unencumbered NYS license as a Physical Therapy Assistant.
• Graduate of an accredited school with an Associate’s degree in Physical Therapy.
• Geriatric experience preferred.
• A current unrestricted driver’s license and dependable transportation.
• Ability to function independently as well as part of a team.

Responsabilidades

• Provides Physical Therapy plan of treatments and modalities under the supervision of a licensed Physical Therapist.
• Communicate tactfully with residents, staff, families, visitors, and the public.
• Communicate with residents with visual, speech, auditory, cognitive, physical and emotional impairments.
• Train/supervise and discipline subordinate staff.
• Relate information regarding a resident’s condition.
• Assist in planning, organizing and implementing department goals and procedures.
• Documents resident progress toward Physical Therapy goals.
• Completes Daily Activity Schedule and progress notes per facility policy.
• Provides resident, staff and family education as directed.
• Complies with Federal and State regulations related to rehabilitation therapy.
• Completes all documentation to comply with professional standards and regulatory requirements.
• Assists in ordering necessary equipment for the facility and residents.
• Maintains confidentiality of facility and resident information.
• Reports all instances of potential resident abuse, neglect, and misappropriation of resident property immediately.
• Follows sanitation and infection control policies and procedures.
• Keeps Facility Rehabilitation manager and Physical Therapist informed and updated appropriately.
