Job summary

Beyond Home Health Care in New Jersey is seeking a licensed Physical Therapist to provide in-home and community-based PT services to adults with developmental disabilities.

The PT will evaluate mobility, strength, balance, gait, and functional movement, and deliver person-centered interventions to maximize physical independence, prevent secondary complications, and support community participation, in accordance with NJ DDD guidelines.

Qualifications

• Active New Jersey PT license issued by the NJ Board.
• Graduate of a CAPTE-accredited PT program (DPT or equivalent).
• Minimum of 1 year clinical experience preferred; new grads considered.
• Current CPR/BLS certification.
• Clean criminal background check and fingerprinting per NJ DDD requirements.

Responsibilities

• Conduct in-home and community PT evaluations and develop individualized treatment plans.
• Deliver one-on-one therapy in homes and community settings.
• Document services per DDD standards with progress notes and outcomes.
• Collaborate with support coordinators, families, caregivers, and the team.
• Provide caregiver training and home programs to reinforce goals.
• Maintain billing records consistent with DDD rates and authorizations.
• Complete required DDD trainings (Pre-Service, Danielle's Law, etc.).
