Key Responsibilities

• Conduct thorough assessments and evaluations to understand clients’ mental health needs and develop tailored treatment plans.
• Engage in consistent communication and collaboration with supervisors, direct care staff, and family members to monitor the progress of individuals under your care.
• Create and provide a monthly schedule detailing appointments, visits, and observations, ensuring transparency and accountability in your work.
• Plan and coordinate home visits and in-home observations, conducting chart reviews and staff interviews to gather essential data and insights for your evaluations.
• Ensure the effective implementation of treatment plans, adjusting methodologies and techniques as necessary to align with clients’ evolving needs.
• Actively participate in Interdisciplinary Team Meetings and contribute to program enhancements, bringing insights from your practice to discuss collaborative care strategies.
• Maintain accurate and up-to-date records, ensuring documentation is accessible and comprehensive for clients and relevant stakeholders.
• Organize training sessions for staff to promote competence in executing program plans and familiarity with documentation practices.
• Provide ongoing support through follow-up interviews and home visits, reinforcing positive outcomes and sustained engagement with clients.
• Serve as a liaison with community resources, forging connections with local mental health agencies to facilitate comprehensive support for clients.

Patient Population / Client Focus

In your role as a Licensed Professional Counselor, you will work with a diverse clientele, including adults and youth from various backgrounds. Your approach will be fundamental in addressing a broad spectrum of mental health concerns, including anxiety, depression, trauma, family dynamics, and substance abuse disorders. Your ability to empathize and tailor your strategies to meet individual needs will be key in promoting positive therapeutic outcomes.

Qualifications

• A Master's or Doctorate degree in Counseling, Psychology, or a related field is required.
• Current and unrestricted licensure as a Professional Counselor (LPC) in the State of Arizona.
• A minimum of two years of experience in counseling, preferably within a clinical context.
• A strong foundation in legal and ethical practices regarding mental health treatment.
