Job summary

Caringhand Home Health Solutions, Inc is seeking an LPN/LVN to join our field-based team on a per diem basis in California. Travel to patients’ homes is required; this is not telehealth.

Compensation is $45–$55 per visit, with opportunities to convert to full-time based on performance. Responsibilities include direct patient care under the Nurse Practice Act, implementing care plans, educating patients and families, and documenting care.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• Current CPR certification with AHA.
• Valid driver’s license and reliable insured vehicle.
• 3+ years of nursing experience; home health or med/surg background preferred.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team to communicate patient needs to the RN or Clinical Supervisor.
• Travel to patients’ homes and use supplies safely.

• Perform other duties as required for patient services.
