Responsibilities

Carlyle Senior Care of Williston is looking for several LPNs to join our team! We are looking for motivated and caring individuals who share in our desire to provide the best long-term care possible. We are currently offering a $5,000 Sign-On Bonus to Full Time LPNs.

Shifts Available

• Full Time Night Shift – works 3-12 hour shifts per week from 7 pm-7 am and is eligible for full benefits package including paid time off.
• Part Time Day or Night Shift – works 1-2 shifts per week from 7 am-7 pm or 7 pm-7 am and is eligible for paid time off.

Responsibilities

• Make accurate and ongoing data collection of the resident’s status and respond appropriately
• Document accurate and ongoing data collection of the resident’s status. Document patient care which reflects nursing interventions, resident’s response to the care provided, resident’s needs, problems, capabilities, limitations and progress toward goals
• Communicate with physicians regarding resident’s needs and changes in their conditions
• Collaborate with other facility staff members to ensure the residents’ well-being
• Perform direct patient care utilizing the established policies and procedures of the facility. Provide personal care to provide comfort and well-being to the residents, acknowledging physiological and psychological needs
• Demonstrate ability to handle emergency situations in a prompt, precise and professional manner
• Provide a thorough, concise report at the change of shift. Communicate pertinent information to the appropriate person in a timely manner. Accurately transcribe physician orders according to the facility’s policies and procedures
• Maintain a clean, neat and safe environment for the facility’s residents and staff. Ensure that supplies and equipment necessary for patient care are stored in an organized and efficient manner
• Assist residents and family members with concerns and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner
• Follow proper reporting procedures for actual or potential accidents and/or incidents
• Administer medications to residents in an accurate, timely manner. Monitor and document medication for residents for effectiveness of drugs and occurrence of adverse effects
• Follow care plans related to each resident’s needs and his/her comprehensive assessment
• Identify resident problems and emergency situations and initiate emergency care and life-saving measures
• Observe all facility safety policies and procedures
• Observe infection control procedures
• Follow Residents Rights policies at all times
• Consistently present yourself to work with an appropriate, professional appearance
• Consistently work cooperatively with residents, residents’ representatives, facility staff, physicians, consultants, and ancillary service providers
• Perform all duties assigned in an effective, cooperative, timely and professional manner
