Job summary

Merakey seeks a Licensed Practical Nurse for a Visiting Nurse role serving Bucks, Montgomery, and Philadelphia counties. You will travel to group homes, perform clinical assessments, administer medications, and update MARs and logs.

This position offers new shift differentials and a schedule that includes daytime and evening hours. You will collaborate with healthcare professionals to coordinate care, educate residents and families, and respond to medical emergencies as needed.

Qualifications

• Current LPN license in PA.
• Preferably 1 year of LPN experience.
• Driver's license with reliable transportation.
• Ability to lift up to 50 lbs occasionally, and up to 20 lbs frequently.

Responsibilities

• Visit group homes to deliver medications and update MARs/Treatment Logs.
• Conduct nursing assessments and identify health issues; report findings to nursing supervisor.
• Review physician orders and administer medications accurately.
• Coordinate care with health professionals while keeping the supervisor informed.

Earn:$32 / hour + Enhanced Pay Eligible.

NEW Shift Differentials

• Monday - Thursday | 2:30 PM - 7:00 AM + $4.00 per hour
• Friday 2:30 PM - Saturday 7:00 AM + $4.00 per hour
• Saturday 6:30 AM - Monday 7:00 AM + $6.00 per hour

Shift:M-F noon - 8:00 PM or 12:30 PM - 8:30 PM

As a Licensed Practical Nurse (Group Home Visiting Nurse) at AVS Merakey, you'll provide direct nursing care to individuals with developmental and physical disabilities. Your responsibilities will include traveling to our group homes, clinical assessments, monitoring acuity, teaching staff on residents care plan and medical needs, delivering medications to group homes, updating MAR and Treatment logs.

Essential Job Functions:

• Visit group homes, deliver medications, update MARs and Treatment Logs, conduct nursing assessments on residents to identify for signs and symptoms of illness, and report findings to the nursing supervisor.
• Identify health issues and follow through to resolution, while documenting all actions.
• Review, transcribe, and administer physician's orders accurately.
• Collaborate with various healthcare professionals and specialists to coordinate care, while keeping the nursing supervisor informed.
• Provide health education to residents and their families.
• Conduct effective in-service training for staff to ensure compliance with protocols and procedures.
• Utilize the least restrictive approaches to behavior management and demonstrate knowledge of psychiatric emergencies.
• Respond effectively to emergency situations in a calm, constructive, and positive manner.
• Perform general nursing duties, including traveling to group homes as a visiting nurse, conduct quarterly physical assessments, and monthly breast exam.

The ideal candidate will possess the following qualifications:

• Current license as an LPN in the Commonwealth of PA.
• Preferably, at least one year of experience as an LPN.
• Genuine interest in working with individuals with physical and developmental disabilities.
• Possession of a current and valid PA driver's license, along with reliable transportation.
• Physical capability to comfortably exert up to 50lbs occasionally, and/or up to 20lbs frequently, and/or up to 10lbs continuously.
