Job summary

Merakey AVS is seeking an LPN Case Manager to join our team in Crafton, PA. You will coordinate the medical portion of the plan of care for a designated caseload of residents, utilize a collaborative approach with the health care team to implement the plan, and serve as the nursing liaison across disciplines to ensure timely, appropriate care.

The ideal candidate has an active PA LPN license, and preferably two years hospital experience, with a genuine interest in supporting adults with IDD.

• Genuine interest in working with individuals with physical and developmental disabilities.

We are seeking an LPN Case Manager to join our team in our Crafton, PAlocation.

Earn $29 per hour with benefits or $31 per hour with the selection of enhanced pay and scaled back benefits.

Schedule: M - F 8:30 AM - 9:0 PM

Merakey AVS is a non-profit organization which provides homes and a full range of services to adults with all levels of intellectual and developmental disabilities (IDD). The majority of the nearly 800 individuals that we support are diagnosed with severe or profound IDD.
