Merakey is seeking a Licensed Practical Nurse to join our Adult IDD team. The role begins at our Warminster facility and includes travel to group homes across Bucks, Montgomery, and Philadelphia counties to provide direct nursing care and coordinate care plans.

The nurse will perform nursing assessments, administer medications, update MARs, educate staff, and collaborate with healthcare professionals to ensure high-quality care for residents with developmental and physical disabilities.

Qualifications

• Graduation from an accredited practical/vocational nursing program.
• Current license as an LPN in the Commonwealth of PA.
• Preferably, at least one year of experience as an LPN.
• Possession of a current and valid PA driver's license, along with reliable transportation.
• Physical capability to lift up to 50lbs occasionally, up to 20lbs frequently, and up to 10lbs continuously.

Responsibilities

• Visit group homes, deliver medications, update MARs and Treatment Logs, and conduct nursing assessments.
• Identify health issues and document actions to resolution.
• Review and administer physician's orders accurately.
• Coordinate care with healthcare professionals and report to nursing supervisor.
• Provide health education to residents and families.
• Conduct in-service training for staff on protocols and procedures.
• Respond to psychiatric emergencies and maintain composure in emergencies.
• Travel to group homes as a visiting nurse and perform quarterly physical assessments and monthly breast exams.

Job description

We are seeking aLicensed Practical Nurse to join our Adult IDD team. Each day will start at our facility in Warminster and then work at our group homes located in Bucks, Montgomery, and Philadelphia counties.

As a Licensed Practical Nurse (Group Home Visiting Nurse) at AVS Merakey, you'll provide direct nursing care to individuals with developmental and physical disabilities. Your responsibilities will include traveling to our group homes, clinical assessments, monitoring acuity, teaching staff on residents' care plan and medical needs, delivering medications to group homes, updating MAR and Treatment logs.

Earn:$32 / hour + Enhanced Pay Eligible.

New Shift Differentials

• Monday - Thursday | 2:30 PM - 7:00 AM + $4.00 per hour
• Friday 2:30 PM - Saturday 7:00 AM + $4.00 per hour
• Saturday 6:30 AM - Monday 7:00 AM + $6.00 per hour

Shift: M-F noon - 8:00 PM or 12:30 PM - 8:30 PM.

Essential Job Functions:

• Visit group homes, deliver medications, update MARs and Treatment Logs, conduct nursing assessments on residents to identify for signs and symptoms of illness, and report findings to the nursing supervisor.
• Identify health issues and follow through to resolution, while documenting all actions.
• Review, transcribe, and administer physician's orders accurately.
• Collaborate with various healthcare professionals and specialists to coordinate care, while keeping the nursing supervisor informed.
• Provide health education to residents and their families.
• Conduct effective in-service training for staff to ensure compliance with protocols and procedures.
• Utilize the least restrictive approaches to behavior management and demonstrate knowledge of psychiatric emergencies.
• Respond effectively to emergency situations in a calm, constructive, and positive manner.
• Perform general nursing duties, including traveling to group homes as a visiting nurse, conduct quarterly physical assessments, and monthly breast exam.
