True Kare Home Health Inc is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our growing team in the Riviera Beach area. Responsibilities:

• Provide direct patient care in home settings, including wound care, medication administration, and vital sign monitoring
• Develop and follow individualized patient care plans under the supervision of a Registered Nurse or physician
• Observe and document patient conditions, reporting any changes to the supervising RN or care team
• Educate patients and family members on care procedures, medications, and health management
• Coordinate with multidisciplinary healthcare teams to ensure continuity of care
• Maintain accurate and timely clinical documentation in compliance with home health regulations
• Assist with patient assessments and contribute to care plan updates as needed

Requirements:

• Active Licensed Practical Nurse (LPN) license in the state of Florida
• Prior home health, long-term care, or clinical nursing experience preferred
• Strong knowledge of wound care, IV therapy, and medication management
• Excellent communication and interpersonal skills for patient and family interaction
• Reliable transportation and valid driver's license for home visits in the Riviera Beach area
• CPR/BLS certification required
• Compassionate, detail-oriented, and able to work independently in a home health environment
