QuadCare Home Health Agency in Palm Beach County is seeking a heart-driven Licensed Practical Nurse to join our growing team in Boca Raton.

Qualifications

• Active LPN license in Florida (Compact license accepted).
• 1+ year of home health or clinical nursing experience preferred.
• Current CPR/BLS certification.
• Valid Florida driver's license and reliable transportation.
• Proof of TB test, required immunizations, and health clearance.
• Strong communication skills and a patient-first mindset.

Responsibilities

• Deliver skilled nursing visits including wound care, catheter care, and medication administration
• Monitor patient vitals and health status, flagging changes to the supervising RN or physician
• Educate patients and families on medications, diagnoses, and how to thrive between visits
• Implement individualized care plans under RN supervision and collaborate with the full care team

• Keep documentation accurate and timely — because great care deserves a great paper trail
