Job summary

True Kare Home Health Inc. is seeking a compassionate LPN to join our Palm Beach Gardens team, delivering skilled home health nursing in patients’ homes across Palm Beach County, FL.

You will monitor patients, administer medications, educate families, and collaborate with RNs and physicians to ensure continuity of care while maintaining meticulous documentation and regulatory compliance.

Qualifications

• Excellent communication and interpersonal skills with patients and care teams.
• Strong knowledge of wound care, medication administration, and patient assessment.
• CPR/BLS certification required.
• Compassionate, self-motivated, and able to work independently in a home setting.

Responsibilities

• Provide skilled nursing care to home health patients per physician-approved care plans.
• Monitor and document patient health status, vital signs, and progress toward goals.
• Administer medications, wound care, and other treatments as prescribed.
• Educate patients and families on disease management and home care techniques.
• Collaborate with RNs, physicians, and care teams to ensure continuity of care.
• Maintain accurate clinical documentation in compliance with regulations.
• Identify and report changes in patient condition promptly.

Responsibilities:

• Provide skilled nursing care to home health patients in accordance with physician-approved care plans
• Monitor and document patient health status, vital signs, and progress toward care goals
• Administer medications, wound care, and other treatments as prescribed
• Educate patients and family members on disease management, medications, and home care techniques
• Collaborate with RNs, physicians, and interdisciplinary care teams to ensure continuity of care
• Maintain accurate and timely clinical documentation in compliance with state and federal regulations
• Identify and report changes in patient condition to the supervising RN or physician promptly

Requirements:

• Active Florida LPN license in good standing
• Previous home health, long-term care, or clinical nursing experience preferred
• Strong knowledge of wound care, medication administration, and patient assessment
• Excellent communication and interpersonal skills with patients and care teams
• Reliable transportation and valid driver's license for home visits in the Palm Beach Gardens area
• CPR/BLS certification required
• Compassionate, self-motivated, and able to work independently in a home setting
