Job summary

Palm Beach - The Roads Home Health is seeking a compassionate Licensed Practical Nurse (LPN) to join our Home Health team in Florida. The LPN delivers skilled nursing care to patients in their homes under RN supervision and according to the established plan of care.

Responsibilities include monitoring patients, administering medications, wound care, and educating patients and caregivers. Travel to patient homes within the service area is required, with emphasis on safety and documentation.

We are seeking a compassionate, dependable, and experiencedLicensed Practical Nurse (LPN)to join our Home Health team. TheLPNprovides skilled nursing care to patients in their homes under the direction and supervision of a Registered Nurse (RN) and in accordance with the established plan of care.

Responsibilities

• Provide skilled nursing care to patients in their homes according to the established plan of care.
• Monitor and document patients’ conditions, vital signs, symptoms, and progress.
• Administer medications and treatments as ordered by the physician and permitted within theLPNscope of practice.
• Perform wound care and other skilled nursing procedures as assigned.
• Observe patients for changes in condition and promptly report concerns to the supervising RN or appropriate clinical team member.
• Educate patients and caregivers regarding medications, treatments, safety, and the plan of care.
• Maintain accurate, timely, and complete clinical documentation.
• Communicate effectively with the RN, physicians, caregivers, and other members of the interdisciplinary care team.
• Follow all agency policies, infection-control procedures, patient-safety standards, and applicable state and federal regulations.
• Travel to patient homes within the assigned service area.

Job Requirements

• Current, valid Florida Licensed Practical Nurse
• Graduate of an accredited physical therapy program
• Minimum of two years of physical therapy work experience
• Current CPR/BCLS certification
• Current health certificate, physical exam, and TB clearanceValid driver's license, reliable vehicle, and current auto insurance; ability to pass a criminal background check
