Job summary

The Roads Home Health Care Inc seeks a compassionate Licensed Practical Nurse (LPN) to join our Home Health team. The LPN provides skilled nursing care to patients in homes under the direction of an RN and the established care plan.

Responsibilities include administering medications, wound care, and documenting care while coordinating with RNs and physicians. Travel within the service area is required; strong judgment, communication, and patient education are essential.

Qualifications

• Current Florida LPN license and good standing.
• CPR/BCLS certification.
• Graduate of an accredited nursing program.
• Two years of relevant home health or clinical experience preferred.

Responsibilities

• Provide skilled nursing care to patients in their homes per plan of care.
• Monitor and document vital signs, symptoms, and progress.
• Administer medications and treatments as ordered by physician.
• Perform wound care and other nursing procedures as assigned.
• Observe for changes in condition and report to RN or clinical team.
• Educate patients and caregivers on medications, treatments, and safety.
• Maintain accurate clinical documentation.
• Communicate with RN, physicians, caregivers, and team members.
• Follow policies, infection control, and patient-safety standards.
• Travel to patient homes within the service area.

Job description

We are seeking a compassionate, dependable, and experiencedLicensed Practical Nurse (LPN)to join our Home Health team. TheLPNprovides skilled nursing care to patients in their homes under the direction and supervision of a Registered Nurse (RN) and in accordance with the established plan of care.

Responsibilities

• Provide skilled nursing care to patients in their homes according to the established plan of care.
• Monitor and document patients’ conditions, vital signs, symptoms, and progress.
• Administer medications and treatments as ordered by the physician and permitted within theLPNscope of practice.
• Perform wound care and other skilled nursing procedures as assigned.
• Observe patients for changes in condition and promptly report concerns to the supervising RN or appropriate clinical team member.
• Educate patients and caregivers regarding medications, treatments, safety, and the plan of care.
• Maintain accurate, timely, and complete clinical documentation.
• Communicate effectively with the RN, physicians, caregivers, and other members of the interdisciplinary care team.
• Follow all agency policies, infection-control procedures, patient-safety standards, and applicable state and federal regulations.
• Travel to patient homes within the assigned service area.

Job Requirements

• Current, valid Florida Licensed Practical Nurse
• Graduate of an accredited physical therapy program
• Minimum of two years of physical therapy work experience
• Current CPR/BCLS certification
• Current health certificate, physical exam, and TB clearance
• Valid driver's license, reliable vehicle, and current auto insurance; ability to pass a criminal background check

Qualifications

• Strong clinical judgment with the ability to synthesize assessment findings into an effective care plan
• Excellent verbal and written communication skills for patient education and interdisciplinary coordination
• Ability to work independently while managing a home visit schedule efficiently
• Compassionate, patient-centered approach to home health and rehabilitative care
• Strong organizational skills and attention to documentation detail
• Prior home health experience preferred
• Bilingual candidates a plus
