Interim HealthCare in Bristol, TN is seeking a Private Duty LPN or RN for 7pm–7am shifts on a 3/4 rotation. This full-time role focuses on one-on-one patient care in the home, offering flexibility, weekend options, and weekly pay.

You'll work under RN supervision, take vitals, administer care, educate families, and coordinate with a care team. We value nurses, support career growth with online training and CEUs, and provide a respectful work culture.

Formación

• Graduate of an accredited practical nursing program.
• Active TN RN or LPN license.
• Current CPR certification.
• Knowledge of state home health regulations.

Responsabilidades

• Provide home-based care to patients under RN supervision.
• Take patient vitals, perform assessments, document progress, and report changes to RN supervisor.
• Assist RN in carrying out patient’s plan of care and teaching self-care techniques.
• Educate family on the patient’s plan of care, medication and home safety.
• Collaborate with interdisciplinary team to coordinate patient care and discharge planning.

As a Home Health Nurse, here’s a big-picture view of what you’ll do:

• Provide home-based care to patients with illnesses, injuries and chronic diseases—often after hospital discharge—under the supervision of an RN
• Work with a team of physicians, RNs, CNAs and aides to deliver quality patient care
• Take patient vitals, do assessments, document progress and report changes to RN supervisor
• Assist RN in carrying out patient’s plan of care and teaching patient self-care techniques
• Educate family on the patient’s plan of care, medication and home safety
• Collaborate with interdisciplinary team to coordinate patient care and plan for discharge

A few must-haves for Home Health Nurses:

• Graduate of an accredited practical nursing program and active RN OR LPN license in TN
• CPR certification
• Knowledge of state and federal home health regulations
• Clinical proficiency in nursing, strong communication skills and compassion
