EveryAge is seeking a compassionate Licensed Practical Nurse (LPN) for a part-time 3p-11p shift on Mondays and Fridays at Providence Place, High Point, NC. The LPN will provide skilled nursing care under the supervision of the RN Supervisor and Director of Nursing, with focus on resident comfort, safety, and regulatory compliance.

Ideal candidates have an unrestricted LPN license, at least one year of nursing experience, and strong collaboration within a multidisciplinary team to support

Qualifications

• Graduate of an accredited School of Practical Nursing.
• Current, unrestricted LPN license in the state of practice.
• Minimum of one year nursing experience.
• Experience in skilled nursing or long-term care setting preferred.
• Current CPR certification required.

Responsabilités

• Provide direct nursing care and medication administration to residents.
• Monitor resident conditions and promptly report changes to the healthcare team.
• Assist in developing and implementing individualized care plans.
• Collaborate with physicians, nurses, caregivers, and families to promote optimal resident outcomes.
• Accurately document care and maintain resident records in accordance with regulatory requirements.
• Promote a safe, positive, and resident-focused environment.
• Ensure compliance with all federal, state, and facility regulations and standards of care.
