Job summary

EveryAge is seeking a Licensed Practical Nurse (LPN) for Providence Place in High Point, NC. The role supports resident-centered care under the supervision of the RN Supervisor and Director of Nursing, ensuring quality, regulatory compliance and compassionate service.

The LPN will deliver skilled nursing care, administer medications, document care, and collaborate with a multidisciplinary team to promote resident health and independence.

Qualifications

• Graduate of an accredited School of Practical Nursing.
• Current, unrestricted LPN license in the state of practice.
• Minimum of 1 year of nursing experience; skilled nursing/long-term care preferred.
• CPR certification required.
• Strong clinical assessment and communication skills.
• Ability to work collaboratively within a multidisciplinary team.
• Compassionate, resident-centered care.

Responsibilities

• Provide direct nursing care and medication administration to residents.
• Monitor resident conditions and promptly report changes to the healthcare team.
• Assist in developing and implementing individualized care plans.
• Collaborate with physicians, nurses, caregivers, and families to promote optimal resident outcomes.
