Job summary

Elara Caring in Oklahoma is seeking a Licensed Vocational / Practical Nurse to join our home health care team and deliver compassionate, high-quality care at the patient’s home.

You will follow individualized care plans, monitor progress, coordinate with the care team, and support families. A valid nursing license, 1 year of clinical experience, and ability to lift up to 50 pounds are required; driving to patient homes is necessary.

Qualifications

• Current State License as a Licensed Vocational / Practical Nurse.
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• 1 year of experience in a clinical care setting.
• Ability to frequently lift up to 50 pounds and assist with patient transfers.
• Must have a valid driver's license or reliable transportation to travel to patient homes.

Responsibilities

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes to the care team.
• Help patients and caregivers learn self-care techniques.
• Work with the RN Case Manager to update care plans as needs change.
• Document medication regimens and update medication profiles.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in a hospice or home health environment is preferred
