Job summary

Touchmark on West Prospect is Appleton’s leading full-service senior living community seeking a skilled Licensed Practical Nurse (LPN) to join our Health Services team.

You will provide high-quality, person-centered care, administer medications, observe and document resident conditions, and collaborate with RNs and leadership to support individualized care plans.

Qualifications

• LPN degree from an accredited nursing program required and current state licensure.
• CPR/First Aid certification required or able to maintain per state requirements.
• 1–2 years of experience working with older adults preferred.
• Strong clinical, communication, and organizational skills.
• Compassion, professionalism and commitment to resident-centered care.
• Ability to work rotating weekends and flexible hours as needed.

Responsibilities

• Provide direct nursing care within the LPN scope of practice.
• Administer medications and treatments.
• Observe, assess, and document resident conditions and care provided.
• Communicate effectively with residents, families, and the healthcare team.
• Support individualized care plans for Assisted Living and Memory Care.
• Assist with training and supporting medication aides and new team members.
