Emerald Nursing and Rehab Lakeview in Grand Island, NE is seeking a Licensed Practical Nurse (LPN) to provide direct patient care in a skilled nursing setting. You will assess resident needs, administer medications, perform wound care, and collaborate with the healthcare team to achieve optimal resident outcomes.

The role requires strong communication, adherence to safety protocols, and the ability to work 6pm–6am in a full-time capacity.

Qualifications

• Valid LPN/LVN license in Nebraska or multi-state.
• Graduate of an accredited LPN/LVN program.
• Current CPR/BLS certification.
• Experience in a skilled nursing facility preferred.
• Excellent verbal and written communication, teamwork.

Responsabilités

• Provide direct nursing care to residents, including medications, wound care, and treatments as prescribed.
• Monitor vital signs and progress, report changes to Charge Nurse or DON.
• Assist in development and implementation of individualized care plans.
• Document patient care clearly and accurately; communicate with residents and families.
• Adhere to infection control and safety protocols.
• Provide emotional support and education to residents and families.
• Collaborate with RNs, CNAs, physicians, and therapists for coordinated care.

Job Summary

As a Licensed Practical Nurse (LPN) at Emerald Nursing and Rehab Lakeview, you will play a crucial role in delivering high-quality, compassionate care to our residents. Your primary responsibilities will include assessing patient needs, providing direct nursing care, and collaborating with the healthcare team to ensure the best outcomes for our residents. Work Environment

Work is performed in a skilled nursing facility.

Responsibilities

• Patient Care: Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician.


• Monitoring and Assessment: Monitor residents’ vital signs, document their progress, and report any significant changes to the Charge Nurse or Director of Nursing.


• Care Planning: Assist in the development and implementation of individualized care plans, ensuring they are tailored to meet each resident’s needs.


• Communication: Maintain clear and accurate documentation of patient care, communicate effectively with residents, families, and the healthcare team.


• Compliance: Adhere to all facility policies, procedures, and regulatory requirements, including infection control and safety protocols.


• Support: Provide emotional support and education to residents and their families, helping them understand care plans and health conditions.


• Team Collaboration: Work collaboratively with other healthcare professionals, including RNs, CNAs, physicians, and therapists, to deliver coordinated and comprehensive care.

Qualifications

• Licensure: Valid LPN/LVN license in Nebraska or Multi-state


• Education: Graduation from an accredited LPN/LVN program.


• Experience: Previous experience in a skilled nursing facility or similar healthcare setting preferred.


• Certifications: Current CPR/BLS certification required. Additional certifications may be required based on state regulations.


• Communications: Excellent verbal and written communication skills. Ability to work collaboratively with a diverse team of healthcare professionals and build positive relationships with residents and their families.


• Physical Requirements: Ability to lift and move patients, perform physical assessments, and stand for extended periods.
