Job summary

Emerald Nursing and Rehab Columbus in Columbus, Nebraska is seeking a compassionate Licensed Practical Nurse (LPN) to join our Nursing team. You will provide direct nursing care, assess resident needs, administer medications, and collaborate with physicians and therapists to ensure high-quality, patient-centered care.

This full-time position (6a-6p or 6p-6a) requires Nebraska licensure, CPR/BLS certification, and prior experience in a skilled nursing facility.

Qualifications

• Licensure: Valid LPN/LVN license in Nebraska
• CPR/BLS certification required
• Experience in a skilled nursing facility or similar healthcare setting preferred
• Excellent verbal and written communication skills
• Ability to lift and move patients and stand for extended periods

Responsibilities

• Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician.
• Monitor residents' vital signs, document progress, and report changes to the Charge Nurse or Director of Nursing.
• Assist in developing and implementing individualized care plans tailored to each resident's needs.
• Maintain clear and accurate documentation of patient care; communicate with residents, families, and the healthcare team.
• Adhere to facility policies, procedures, and regulatory requirements, including infection control and safety protocols.
• Provide emotional support and education to residents and families about care plans and health conditions.
• Collaborate with RNs, CNAs, physicians, and therapists to deliver coordinated and comprehensive care.
