Position Summary

Responsibilities
Essential Functions
  • Ensure continuity and coordination of care across inpatient, emergency, and outpatient settings, including discharge planning and follow-up.
  • Build effective relationships with the clinical team, patients, and families to support care and advocate for patient needs.
  • Monitor patient progress and adjust discharge plans as needed, prioritizing high-risk patients.
  • Educate patients and families on chronic illness management, healthcare systems, and available resources.
  • Identify support needs, develop action plans, and assist with access to prescriptions, equipment, and services.
  • Collaborate with team members using IT resources to track and improve patient outcomes and adherence to guidelines.
  • Participate in clinical outcome measurement and ensure patient safety and policy compliance.
Other Functions
  • Demonstrate analytical skills, teamwork, and independence.
  • Provide age-appropriate care based on developmental principles.
  • Maintain awareness of legal, ethical, and regulatory standards.
Qualifications
Education/Training
  • Effective July 1, 2024: Must have a Bachelor of Science in Nursing (BSN).
Licensure/Certification
  • Current RN license in Florida.
  • Current BLS certification.
Experience
