BroadPath is immediately hiring Clinical Care Management Coordinator to work from home! Our Clinical Care Management Coordinator is a member-facing outreach specialist that supports the Clinical Care Team by assisting in member engagement through telephonic outreach, collecting member-specific health data, documentation of member outreach/engagement, and data entry.

Responsibilities

• Initiate outbound calls for care management campaigns to encourage member participation in case management
• Gather information related to members’ social, clinical, and functional status to identify needs
• Complete and enter Health Risk Assessments; initiate care plans based on findings
• Respond professionally to inquiries from members, providers, and regulatory agencies while maintaining confidentiality
• Assist members in locating appropriate medical providers including primary care, therapy, and specialty services
• Act as a resource for both internal and external customers
• Collaborate with RN Case Managers to share member-specific needs, initiate referrals, and support care plans
• Support quality improvement initiatives as needed
• Help members access community-based services such as transportation, housing, and financial aid
• Coordinate referrals to programs and services through a wide network of resources
• Ensure accurate, timely, and consistent documentation of all activities
• Effectively plan, organize, and prioritize daily assignments to meet department goals and deadlines
• Perform other related duties as assigned

Qualification

• Clinical background required; licensure not mandatory
• Proficient in Microsoft Office and able to type at least 35 WPM
• Strong analytical, organizational, and problem-solving skills
• Excellent verbal and interpersonal communication skills
• Ability to manage tasks effectively under pressure
• Demonstrates a professional presence both over the phone and in person
• Consistent attendance and reliability are essential
