Job summary

UP Health System - Portage is seeking a Licensed Practical Nurse (LPN) to join the Wound Care Clinic on a full-time basis. The role involves performing clinic duties, patient assessments, and care under a physician's supervision while ensuring compassionate, comprehensive patient support.

The position requires a Michigan LPN license, BLS certification, and eligibility for influenza vaccination or declination.

Qualifications

• State of Michigan LPN license required.
• Graduation from an LPN or LVN program.
• Basic Life Support certification is required (available upon hire).
• Annual Influenza Vaccination or Declination Required.

Responsibilities

• Collects and documents patient intake data; maintains accurate and complete patient records.
• Provides patient support reflecting emotional, physical, social, cultural, religious, and economic considerations.
• Assists with routine clinic support activities.
• Advocates for the physical and emotional well-being of patients.
• Maintains a safe and therapeutic clinic environment per policy.
• Assists with patient assessments and treatments under physician supervision.
• Performs other duties as assigned.

Schedule: Monday-Friday, 8-hour Day shifts

What we're looking for

Applicants should have a state of Michigan LPN license and have graduated from an LPN or LVN Program. Additional requirements include:
