Job summary

Lifepoint Health, through UP Health System - Portage, seeks an LPN for its Urology Clinic. The role supports patients with a focus on compassion and teamwork in a outpatient setting. Clinic hours are Mon-Fri with a standard day shift, contributing to high-quality patient care within a respected network.

The LPN will collect intake data, assist with treatments under physician supervision, and help maintain a safe, patient-centered environment while coordinating care with the clinic team.

Qualifications

• Must be a graduate of an LPN or LVN program.
• Hold a valid Michigan LPN license.
• Basic Life Support (BLS) certification required (available upon hire).
• Annual influenza vaccination or declination required.

Responsibilities

• Collects and documents patient intake data and maintains accurate records.
• Provides patient support considering emotional, physical, social, cultural and economic factors.
• Assists with routine clinic support activities.
• Advocates for the physical and emotional well-being of patients.
• Maintains a safe and therapeutic clinic environment and assists with patient assessments and treatments under physician supervision.

How You’ll Contribute

• Collects and documents patient intake data. Maintains accurate and complete patient records.
• Provides patient support that reflects considerations for emotional, physical, social, cultural, religious and economic influences.
• Assists with routine clinic support activities.
• Works as an advocate for the physical and emotional well-being of the patient.
• Maintains appropriate, safe and therapeutic environment per clinic policy.
• Assists with performing patient assessments and treatments under the supervision of a physician.
• Performs other job duties as assigned.

What We’re Looking For

Applicants should have a state of Michigan LPN license and have graduated from an LPN or LVN Program. Additional requirements include:
