UP Health System - Portage, part of Lifepoint Health, is seeking a Registered Nurse for the Cardiology Clinic. This full-time position features a Monday–Friday day schedule and 8-hour shifts, joining a supportive nursing team.

You will implement care plans, administer medications, document care, and coordinate with the care team while educating patients and families. A Michigan RN license and BLS are required.

Qualifications

• Current Michigan RN license (or within 90 days of hire).
• Associate degree from an accredited nursing program (bachelor's degree preferred).
• Basic Life Support certification required (available upon hire).
• Annual influenza vaccination or declining vaccination required.

Responsabilités

• Performs plan of care interventions including medication administration, specimen collection, and clinical treatments.
• Documents patient care provided and monitors patient condition with the care team.
• Collaborates across disciplines to coordinate patient care, transfers, discharges and referrals.
• Assesses learning needs and provides patient/family education appropriate to age and culture.
• Advocates for the patient's physical and emotional well-being.
• Delegates care activities to qualified staff.
• Performs prior authorizations for tests, procedures, and medications as required.
• Performs other duties as assigned.

How you'll contribute

A Clinic RN who excels in this role:

• Performs plan of care intervention, including medication administration, specimen collection, clinical treatments, as well as other medical care treatment.
• Documents patient care given.
• Monitors, records, and communicates patient condition as appropriate to care team, physician, nursing home, patient, and family.
• Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs.
• Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances.
• Works as an advocate for the physical and emotional well-being of the patient.
• Delegates patient care activities to appropriately qualified staff.
• Performs prior authorizations on tests, procedures, medications, etc. that are required by the patient's insurance.
• Performs other job duties as assigned.

What we're looking for

Applicants should have a current Michigan RN license (or attain within 90 days of hire) and possess an associate degree from an accredited nursing school (bachelor's degree preferred).Additional requirements include:

• Basic Life Support certification is required (available upon hire)
• Annual Influenza Vaccination or Declination Required
