RiverSpring Living is seeking a Licensed Practical Nurse (LPN) Supervisor to join our team at our RiverSpring Licensed Home Care Services Agency!

The RiverSpring Licensed Home Care Services Agency (LHCSA) provides comprehensive and personalized home healthcare services for older adults to maximize their independence at home. The LPN Supervisor will provide neighborhood leadership and essential nursing services within our Assisted Living Residence (ALR), following agency policies under the supervision of the Registered Nurse and the Director of Nursing.

The RiverSpring Living campus is set on 32 acres overlooking the Hudson River, in Riverdale, NY. RiverSpring offers nursing home care, managed long-term care, assisted living, senior housing, rehabilitation, and specialized services like elder abuse prevention and memory care.

Key Responsibilities

• Oversees neighborhoods and manages Home Health Aides (HHAs) in residents’ care plans
• Provides supervision and direction to Home Health Aides

• Delivers nursing care per agency policies
• Participates in incident reporting and follow-up
• Assists with resident admissions and conducts daily stand-up meetings
• Maintains accurate clinical records and reports

• Assists in preparing and evaluating nursing care plans
• Observes, documents, and reports changes in residents’ conditions
• Performs nursing procedures, including medication administration
• Manages resident medications, including prescriptions and refills
• Assists with assessments of residents’ health needs

• Promotes a positive work environment and staff development
• Addresses staff concerns and enforces organizational policies

Residents Relations & Support

• Ensures a safe and healthy environment for residents
• Provides resident/family education on self-care and care continuity

Qualifications/Skills

• Graduate of an accredited practical nursing program
• Valid NY State LPN license
• 1-2 years of experience in community, acute, or adult care facilities
• Supervisory experience and medication management preferred

Compensation

• $5,000 sign-on bonus (less taxes)
• $40/hour
