Position Details:

• Part Time - Day Shift

Join our compassionate team to provide hands-on nursing care in a welcoming medical office or the comfort of patients' homes, delivering meaningful care within the Licensed Practical Nurse scope of practice in New York State.

Job Responsibilities:

• Perform direct patient care and procedures within the scope of practice of an LPN in various settings including medical offices, homes, schools, and clinics.
• Interview patients, document health history, medications, and symptoms.
• Measure and record vital signs accurately, demonstrating age-specific competencies.
• Perform diagnostic tests such as blood pressure monitoring, lab draws, EKG, PFT, Peak Flow, and CLIA-waived tests; report results to providers.
• Assist providers with examinations and administer immunizations and medications.
• Draw blood, prepare specimens, and manage test orders.
• Maintain infection control standards, clean instruments, and prepare exam rooms.
• Ensure proper documentation, manage referrals, and coordinate appointments and transportation.
• Maintain supplies and medical equipment, and ensure quality assurance.
• Provide patient education on health, treatment, and disease prevention.
• Facilitate prescriptions and communicate test results to patients.
• Participate in meetings, training, and continuing education.

Qualifications:

• Current New York State license and registration as an LPN.
• Minimum of three years' diverse clinical nursing experience.
• Knowledge of PCMH and accreditation processes (preferred).
• CPR certification.
• Ability to work indoors and around ill patients, with physical capability to lift up to 100 lbs.
