Job summary

Hartland Park Health & Rehabilitation in Lexington, KY, is seeking compassionate Licensed Practical Nurses (LPNs) to join our clinical team. We offer full-time, part-time, and PRN opportunities with day and night shifts (7AM-7PM, 7PM-7AM) and access to daily pay.

Responsibilities include administering medications, collaborating with RNs and CNAs, and delivering high-quality resident care while maintaining confidentiality and complying with regulations.

Qualifications

• Licensed Practical Nurse (LPN) licensed in KY with current CPR/BLS.
• Experience in Long Term Care or Skilled Nursing required.
• Positive attitude, reliability and strong clinical judgment.
• Ability to work with interdisciplinary teams.

Responsibilities

• Provide nursing care to meet resident needs per standards and policies.
• Administer medications and treatments with precision.
• Collaborate with RNs, CNAs, and interdisciplinary teams.
• Support residents’ physical, emotional and psychological well‑being.

What You’ll Do:

Provide nursing care to ensure resident needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines

Administer medications and treatments with precision and compassion

• Collaborate with RNs, CNAs, and interdisciplinary teams
• Support residents’ physical, emotional, and psychological well‑being
• Assist with resident comfort.
• Collect samples and monitor catheters. Provide wound care.
• Keep resident patient information confidential, follow all policies and procedures, and adhere to federal, state, local requirements, and professional standards.
• Collect, track, maintain, and review patient medical history, charts, and physical condition, including heart rate, blood pressure, temperature and respiration.
• Help coach staff as necessary.

Requirements:

• Current LPN license in KY
• CPR/BLS certification (or willing to obtain)
• Experience in Long Term Care or Skilled Nursing Required*
• Positive attitude, reliability, and strong clinical judgment
