Job summary

Holston Health & Rehabilitation in Wytheville, VA is seeking compassionate LPNs for full-time, 12-hour shifts (7:00 am–7:00 pm or 7:00 pm–7:00 am). You will provide nursing care per standards and collaborate with RNs and CNAs.

We offer $28.00–$31.00/hr plus shift differentials, access to Daily Pay, comprehensive benefits, and opportunities for advancement. New grads with Virginia LPN licensure are welcome to apply.

Qualifications

• Current LPN license in Virginia.
• CPR/BLS certification or willingness to obtain.
• Experience in LTC/SNF preferred, new grads welcome.

Responsibilities

• Administer medications and treatments per standards and orders.
• Collaborate with RNs, CNAs, and interdisciplinary teams.
• Support residents' physical, emotional, and psychological well-being.
• Provide wound care and monitor patient conditions.
• Maintain confidentiality and adhere to policies and regulations.
• Track and review patient histories, vitals, and conditions.
• Coach and mentor staff as needed.

What You’ll Do:

Provide nursing care to ensure resident needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines

• Administer medications and treatments with precision and compassion
• Collaborate with RNs, CNAs, and interdisciplinary teams
• Support residents’ physical, emotional, and psychological well-being
• Assist with resident comfort.
• Collect samples and monitor catheters. Provide wound care.
• Keep resident patient information confidential, follow all policies and procedures, and adhere to federal, state, local requirements, and professional standards.
• Collect, track, maintain, and review patient medical history, charts, and physical condition, including heart rate, blood pressure, temperature, and respiration.
• Help coach staff as necessary.

Requirements:

• Current LPN license in Virginia
• CPR/BLS certification (or willing to obtain)
• Previous LTC or SNF experience preferred, but new grads welcome!
• Positive attitude, reliability, and strong clinical judgment

Compensation for the role will depend on several factors, including a candidate’s qualifications, skills, competencies and experience and may fall outside of the range shown.
