Job summary

Care Options for Kids is seeking an LPN to provide dedicated pediatric home health care in a one-on-one setting with the patient’s family. You will work closely with a registered nurse and clinical leadership to follow plans of care and ensure high-quality outcomes in the home environment.

Eligible candidates may be considered for a sign-on bonus. The role emphasizes stability, support, and meaningful differences in a child’s life, with a team committed to nurse safety, PPE, and CDC-aligned

Qualifications

• Must have valid Pennsylvania LPN license (or multi-state license).
• TB test or TB blood test.
• Valid BLS CPR card (in-person).
• Valid driver’s license.
• G-tube/trach/vent experience or willingness to train.

Responsibilities

• Provide one-on-one skilled nursing care in pediatric home health.
• Collaborate with RN and clinical leadership to maintain care plans.
• Deliver care with safety, compassion, and consistency.
• Maintain clear communication with family and care team.
