Wellpath / Correct Care Solutions is seeking a part-time Licensed Practical Nurse (LPN) for their intake team in Vancouver, WA. The role involves patient assessments, administering medications, and documentation using standard protocols. Candidates should have graduated from an accredited nursing school and possess a current LPN license. Qualifications

Key Responsibilities

• Under RN supervision, assess patients’ physical, psychological, and social dimensions in both the Health Care Unit and housing units, contributing to individualized treatment planning and consulting with RNs and staff as needed.
• Implement treatment programs directed by Health Care Practitioners, administer medications, including IV therapy (if certified), and perform diagnostic tests according to established protocols and procedures.
• Collaborate in controlled substance counts at the beginning of each shift, adhering to safety techniques, procedures, and standard precautions while responding to codes or health emergencies within standard guidelines.
• Assist in medical or minor surgical procedures, implement nutrition and therapeutic diet plans, and document nursing encounters using SOAP charting, ensuring effective communication with nursing staff, physicians, and other relevant personnel.
• Attend mandatory staff meetings and training sessions, demonstrating a commitment to continuous professional development and adherence to policies and procedures in providing high‑quality patient care.

Experience
