Qualifications

Responsibilities

• Takes an active role in direct customer care.
• Administers medication and/or treatment and documents it for customers.
• Responsible for clinical documentation related to customer activity.
• Develops and implements individual nursing care plans; participates in care plan meetings.
• Counts and records class drugs at shift changes.
• Communicates data to Unit Manager/Supervisor and/or Physician.
• Reviews and updates communication boards; monitors alerts during shifts.
• Provides emotional and physical comfort and safety to customers.
• Assists physician in rounds and examinations when needed.
• Supervises CNAs and other LPNs in providing care per guidelines.

Beacon Health Management is hiring a Licensed Practical Nurse (LPN) to work at Hazlehurst Court Care and Rehab providing both short term rehabilitation and long-term care.

Position Summary

In conjunction with the Unit Director, the Licensed Practical Nurse utilizes a general understanding of the principles of Nursing in the development of and implementation of individualized Nursing Care Plans to ensure that the needs of the customers are met. He/she assists in the orientation of and supervision of nursing personnel, attends to the daily operations of the unit on per shift, unit level, and assumes responsibilities of a leadership role as needed.

Licensed Practical Nurse (LPN) RESPONSIBLITIES

• Takes an active role in direct customer care
• Administers medication and/or treatment and documentation of same to customers
• Responsible for clinical documentation related to customer activity
• Assists in developing and implementing individual nursing care plan for each customer. Participates in care plan meetings
• Counts and records class drugs when beginning and ending shift
• Communicates pertinent data to Unit Manger/Supervisor and/or Physician
• Reviews and updates communication board; as well as monitors and addresses alerts frequently throughout shift
• Provides for emotional and physical comfort and safety of customers
• Assist Physician in rounds and examination of customers, as needed
• Supervises and coordinates activities of Certified Nursing personnel and other LPN's in providing direct customer care in adherence to State, Federal and Corporate guidelines
• Enhances education by attending all mandated in-service programs
• Puts Customer Services First: Ensures that customers and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals' needs and rights

Educational/Vocational Requirements

• The Licensed Practical Nurse must be a graduate of an approved school for Practical or Vocational Nursing and must be currently licensed by the State Board of Nursing.
• Experience is preferred in geriatric or rehabilitative Nursing
