Job summary

Traditional Home Health Care Pediatric Division is seeking a compassionate Licensed Practical Nurse (LPN) to join our team. You will provide one-on-one in-home care to pediatric and adult patients, following a plan of care and administering prescribed medications and therapies.

We offer weekday and weekend shifts, full-time and part-time schedules, with competitive pay ($28-$32 per hour) and weekly direct deposit. Location: Duryea, PA area.

Qualifications

• Licensed Practical Nurse (LPN) required.
• Ability to provide one-on-one in-home care.
• Documentation of care plans and deliveries.
• Administration of prescribed medications and therapies.
• Assessments and coordination of care with clinicians.

Responsibilities

• One to One patient care in home/school environment.
• Implement and document the plan of care to ensure quality and continuity of care for patient.
• Administration of prescribed medication, treatments and therapies.
• Patient assessments; coordination of care.
• Reports any changes in the client condition to Clinical Manager and physician.
• Health promotion, teaching, and training of family members.

Duryea, PA: Monday Friday 8:30am-2:30pm for school accompaniment beginning 8.25
